How does Case management work? Can you tell us about ACM Care?
We specialize in medical case management for seniors and disabled adults. We meet with the family and/or the client to join them in their health journey. we help them to understand what is going on in their own health and medications. We assist them with doctors appointments. Assisting them with organizing their medications and coordinating their care. When it is time to transition from with their home, we can help them find the best option and make the best decision.
Can you explain the charitable side of ACM Care and how did that aspect start?
Our founder Julie Hess started ACM Care. She had a family member that needed help. She saw that they needed help with guidance in their health care. Julie felt that is is important to help people regardless of their ability to pay. We can help low income people navigate their health care.
Tell us about the services you provide and what makes ACM Care unique?
We try to provide peace of mind for families and our clients. We help people who might have a family member out of state and they are not sure who to go to for help. When someone goes into the hospital sometimes they get new medications. They need advise on how meds work together and navigating the health care. We are an advocate for the client and provide resources to guide them. We can go to the doctors appointment as a professional, and ask the right questions to determine the right steps in the clients care. We guide them with our professional opinion.

Who is your ideal client at ACM Care?
A disabled person, senior, or veteran that has been struggling more lately. Maybe they have been going to hospital or urgent cares more frequently. Or if you go to visit your loved one and you notice that a decline, please contact us to get a general analysis of what is going on. We come in as a third party and help with the situation.
