How it works:
- Call us or fill out our online form to determine your eligibility.
- We will then submit your assessment to Medicaid.
- Medicaid will send a nurse to your home to conduct an in-person interview and approve you for your services.
- Building Bridges will assist you with everything afterwards. We will address all your questions and concerns and help you choose an aid.
