Services Provided by Forget Me Not Adult Day Services
Cognitive Stimulation Therapy
Cognitive Stimulation Therapy (CST)is an intervention for dementia patients involving enjoyable activities to stimulate thinking, concentration, and memory in a social setting. Our staff is CST certified and trained through SLU.
Physical Activities
Engage in client-modified physical activities and games, along with 1 hour of daily exercise, dance classes, and more. Our director is a trained and certified APDA fitness professional.
Group Activities
We’ve organized some awesome group activities aimed at boosting socialization, recreation, and cultural engagement. These activities will help keep everyone feeling great and functioning at their best and provide support during our group meetings.
Therapeutic Sound & Music Activities
At our Forget Me Not Adult Day Services, we provide a range of music activities, including amateur performances, therapeutic sound, karaoke, meditation., etc. Music is a powerful tool that promotes wellness, positivity, happiness, reminiscing, and conversation among our participants.
Pet Therapy
Volunteers and certified therapy dogs from local organizations visit monthly to provide pet therapy. This has been known to reduce agitation and negative behaviors associated with dementia, helping those with cognitive issues to feel more relaxed.
Alzheimer’s Care
We provide professional supervision and observe participants while offering support with daily activities in a secure environment equipped with door safety and sound alarms.
Outreach Programs
Join us for social community outings, engaging educational visits, enriching library visits, and heartwarming intergenerational activities with students from a local school.
Cognitive Activities
Engage in the creativity of arts and crafts, stimulate your mind with trivia, board games, word search, DIY projects, reminiscence therapy, and more inspirational cognitive activities!
Well-balanced meals
We are pleased to offer a selection of healthy meals, including breakfast, lunch, and two nutritious snacks. We are fully equipped to accommodate any dietary restrictions you may have.
Our Staff
Our professional staff includes licensed and certified COTAs, LPNs, RNs, and direct support professionals who undergo extensive screening and are trained to work with seniors with dementia and
Parkinson’s.
About Nicole Collora- Program Director
There is a beautiful symbolism behind forget-me-not flowers, representing true love and respect. These flowers hold a deep significance to me, as I lost my mother to Dementia in April 2019. Her
journey taught me invaluable life lessons about the power of love, communication, and the impact of music. Her legacy lives on through the Forget Me Not Adult Day Services, where we strive to honor her memory each day.
Driven by my passion to support seniors coping with dementia, my goal is to provide them with purpose, dignity, and independence while enhancing their quality of life. Our program aims to create a safe and nurturing environment for your loved ones, while also offering caregivers the much-needed respite they deserve from the demands of caregiving.

We understand that it’s important for everyone to feel comfortable. That’s why we offer a free 5-hour trial day for participants to get to know our program. This gives us the chance to address any
questions, understand the participant’s needs, and review the required documentation. We want to make sure it’s the perfect fit for everyone involved.

A try out day allows the participant to attend our program for a partial day (5 hour max). This allows the participant, family, and staff to “try out” one another and assure our program is the perfect fit for everyone! We will take this time to answer any questions you may have, understand the care needs of the participant and review the necessary documentation.
