- Be a Traditional Medicare beneficiary
- Have a confirmed dementia diagnosis, such as Alzheimer’s, LBD, FTD, or vascular dementia
- Be enrolled in Medicare Parts A and B, with Medicare as their primary payer
- Not be enrolled in Medicare Advantage, including Special Needs Plans, or PACE programs
- Not have elected the Medicare hospice benefit
- Not be a long-term nursing home resident

Additionally, the program provides Medical Oversight with clinical recommendations and ongoing care management led by a multidisciplinary team. We collaborate closely with your existing healthcare providers to seamlessly coordinate and enhance the quality of your care, making sure that all aspects of your health are effectively managed.
Will additional documentation or referrals be required?
Yes, additional documentation or referrals may be required for patients to enroll in the GUIDE program. Healthcare providers will need your Medicare cards and some medical records or information that confirms a patient’s diagnosis of dementia or cognitive impairment. In some cases, a formal referral from the patient’s primary care physician or specialist may also be necessary to initiate participation in the program.
Am I eligible for GUIDE?
Eligibility for Medicare’s GUIDE program focuses on individuals diagnosed with dementia or cognitive impairments, along with their caregivers. Beneficiaries must be enrolled in traditional Medicare and meet specific medical criteria to qualify for the program’s services.
